    CarePlanners is a team of senior care advisors in clinical services, advocacy, geriatric care management, social work, medical billing and finance who work collaboratively with families and professionals to coordinate services for your aging loved ones.

    Alan Blaustein founded CarePlanners in 2011 following his battle with thymic cancer and his firsthand experience with the inefficiencies of the healthcare system. His dear friend and co-founder Dr. Nancy Snyderman is a surgeon and broadcast journalist. She’s also a caregiver.  About Us

    CarePlanners primary mission is to help you and your loved one get the care they deserve and ensure your coordination needs are appropriately met.  We will ensure your loved one is safe and properly cared for in their living situation.  We also help in informed decision making and expert coordination of  services.  CarePlanners is also there in case of any crisis or emergency.  And available to help in financial and legal questions and resolution.

  5. How does CarePlanners help in an emergency?

    CarePlanners is not an emergency medical service and should not be your first call in a true emergency situation – instead you should dial 9-1-1.

    CarePlanners will be able to help you and your loved ones deal with urgent medical and insurance issues, helping you sort through the decisions you need to make in the aftermath of an emergency or crisis.  Also through our proprietary online tools you and your family will have access to a relevant electronic record that can be used to provide emergency providers.

  8. Are your Careplanners all nurses? What's their background and training?

    Our Careplanners are trained professionals from across the healthcare spectrum. They are registered nurses, nurse practitioners, social workers, Medicare specialists, insurance billing and claims experts, and other experts.

    We have set the highest standards for our Careplanners based on experience and commitment to customer service and client satisfaction. Each Careplanner has completed our extensive training program and they are evaluated on an ongoing basis to ensure they are providing the highest level of service.

  9. Does CarePlanners provide 24-hour coverage?

    Currently, CarePlanners can be reached 24/7 via our online tools.  If reaching us after hours, a message can be left and a representative will return your phone call upon the start of business the next day.

  10. Are CarePlanners services available nationwide?

    Currently we are focused on the tri-state area of New York-New Jersey-Connecticut. However some healthcare issues and questions can be resolved by Careplanners that are not in your immediate geographic area. For example, most insurance billing and claims issues and coordination of care can be done from anywhere.

    Our longer-term goal is to create a broad, national network of Careplanners to provide you with the local information and resources that you need.

  11. What is a CarePlanner membership?

    By joining CarePlanners as a member you and your loved one will be assigned to a dedicated Careplanner. He or she will perform your in home visits and monitor your ongoing progress and issues.  The Careplanner will also be responsible for regular communication with you and your loved ones as long as you are a member.  Your membership also gives you access to our online tools and proactive reminders.  If you are in need of additional services, you will also be able to receive substantial discounts off our normal hourly rate.

    The only people who have access to your information are the people you choose to share it with, such as your Careplanner and family members. You are also able to revoke access to anyone previously linked to your account.

    CarePlanners uses industry-leading privacy practices and security tools to protect consumer data from security breaches, including the encryption, backup and emergency restoration of data. Through both our on-call and our online platforms, you can be assured that your personal information is handled with confidentiality and security.

  16. Do I have to share sensitive information, such as my social security number or health insurance information? Why does CarePlanners need this?

    Your Careplanner will ask for your health insurance information, including your social security number and your medical records, among other things. We understand that this is sensitive and private information, so we don’t require you to provide anything. But keep in mind, the more information you provide, the better we can serve you.

  17. How do I send CarePlanners confidential information? Is everything handled electronically?

    You have the option of sending your confidential information electronically or by mail. Should you choose to send your information electronically, we use secure encryption technologies to keep the information safe and protected.

    Yes, upon your request and with your permission, your Careplanner can speak on your behalf to your physicians, insurance companies and any other healthcare providers.

    Both platforms work together to make managing your loved one’s care more efficient.  CarePlanners provides a fully integrated system in which information you enter in your CareCenter on CarePlanners.com is also available, with your permission, to your Careplanner through the telephonic service. Conversely, you can view any information entered in by your Careplanner by logging in to your CareCenter.

    The CarePlanners.com online platform is designed to provide personalized healthcare information and recommendations for you, based on the information you provide and update in your profile. The more information you add, the more useful the tools become.

    The careplanners telephonic service is meant to work together with CarePlanners.com. The website includes CareCenter or dashboard to provide you with quick access to your healthcare information and files. Your Careplanner can help you get started updating your profile, and you can add to the profile as needed, keeping it updated with your medical history, medical provider contact information, and your medications.

    The more information you provide, the more personalized and useful the online dashboard will be. For instance, CarePlanners can provide reminders about upcoming preventative services such as immunizations and checkups. We can provide alerts when administrative issues need attention, such as Medicare enrollment or changes in healthcare coverage for young adults.

    Yes, when you work with a Careplanner, they will update your information on your CareCenter while you are working with them so the profiles on your CareCenter are current. You can also upload whatever documents you want to store within your profile.

  23. Do CarePlanners do in home consultations?

    Yes, CarePlanners will provide you with a Senior Care Advisor (Careplanner) that will meet with you and your loved one in the comfort of your own home setting.  The initial visit will include a comprehensive holistic assessment to identify the important care priorities for you and your family.  The Careplanner will routinely monitor progress and provide timely updates and reminders, and return to the home for additional evaluation as needed.
